Selecting a PCP and finding doctors and facilities.

Your plan has convenient access to high-quality, connected care at conveniently located Kaiser Permanente facilities. You also get access to a statewide and national provider network, giving you in-network access to hospitals, clinics, and providers nationwide.

You have many provider options for care.

When seeking care at home or traveling around Colorado, you can choose to see an available Kaiser Permanente provider or other network providers. When you travel outside of Colorado, you have in-network access to all Kaiser Permanente providers and facilities. Additionally, you have nationwide network coverage through the MultiPlan Medicare Advantage Network. You also have access to see any licensed provider across the country who accepts Original Medicare.

Kaiser Permanente Providers

You can find a Kaiser Permanente Provider in the print or online version of the Senior Advantage Choice PPO Provider Directory or call Member Services toll-free at 1-800-476-2167 (TTY 711), 8 a.m. to 8 p.m., 7 days a week.

You can learn about Kaiser Permanente doctor profiles, which include information on education, languages spoken, hobbies, and more. You can select a primary care provider (your personal doctor), who manages your specific care, online or by calling Member Services.

You can schedule appointments online or call the Appointments and Advice Line at 303-338-4545.

Colorado Network Providers

You can find network providers within Colorado who participate in the Senior Advantage Choice PPO Network in the print or online version of the Senior Advantage Choice PPO Provider Directory or call Member Services toll-free at 1-800-476-2167 (TTY 711), 8 a.m. to 8 p.m., 7 days a week.

Once you locate or confirm your current provider is in the network, call the provider office directly to schedule an appointment. Let the provider know they are a network provider in the Kaiser Permanente Senior Advantage Choice PPO plan. Bring your Senior Advantage Choice PPO Member ID card, which has important information to help you check in for your appointment, like the name of the health plan and health record number.

MultiPlan Medicare Advantage Network Providers

When you are seeking care outside Colorado, you have access to the MultiPlan Medicare Advantage Network Providers. To find a provider, visit the online Senior Advantage Choice PPO Provider Directory. The MultiPlan Medicare Advantage Network name will be visible on each provider profile.

You will need to call the provider’s office directly to schedule an appointment. Let the provider’s office know that you have a Kaiser Permanente Senior Advantage Choice PPO plan that allows you access to the MultiPlan Medicare Advantage Network.

Bring your member ID card, which includes the MultiPlan Medicare Advantage Network located on the back of the ID card.

Out-of-Network Providers

You also have access to see any licensed provider across the country who accepts Original Medicare.

You will need to call the provider’s office directly to schedule an appointment. Let the provider’s office know that you have a Kaiser Permanente Senior Advantage Choice PPO plan that allows you access to out-of-network providers.
