The Preferred Provider Organization (PPO) plan combines two provider network options, First Health® and the Cigna Healthcare PPO Network, along with a Non-Participating Provider Tier to offer your employees more care options in one plan
Participating Provider Tier
With the Participating Provider Tier, members have two network options depending on where they receive services:

In Kaiser Permanente States: The First Health Network provides access to care in the Kaiser Permanente states of CA, CO, GA, HI, MD, OR, VA, WA, and DC.
Non-Kaiser Permanente States: The Cigna Healthcare PPO Network* only provides access to care in non-Kaiser Permanente states.
*Disclaimer: The Cigna Healthcare PPO Network refers to the health care providers (doctors, hospitals, specialists) contracted as part of the Cigna Healthcare PPO for Shared Administration.
Non-Participating Provider Tier
Members can receive services from any licensed provider or facility they choose (except those already in the Participating Provider Tier).
